LIABILITY RELEASE

I do hereby permit our son/daughter to participate in all aspects of cheerleading and training including, but not limited to, gymnastics, tumbling, trampoline training, physical conditioning, and other physical activities while a student at Cheer One/Galaxy Allstars. By granting permission of said student to participate in this program, I/we release Cheer One/Galaxy Allstars, its owners, employees, volunteers, and other affiliated instructors from all liabilities that may occur from any injury, including death, that may occur as a result of your child’s participation in any aspect of Cheer One/Galaxy Allstars, that involves height and motion, and that these activities can result in serious injury, disability, and even death. I/we waive any claim to compensation for injury, disability, or death that may occur to the above mentioned student that may result from participation in any Cheer One/Galaxy Allstars program. I/we have read and fully understand this release and I/we allow the registered mentioned student to participate in all activities at the Cheer One/Galaxy Allstars. I/we have read and agree to the terms above.
